Our office strives to provide every patient with the same ‘Standard of Care’ recommended by the American Dental Association (ADA).At each cleaning visit we strive to complete the following; Prophylaxis (cleaning), Exam, Fluoride Varnish, Bitewing X-rays (1-4), and Intraoral Periapical X-rays (2-3) OR a Panoramic image which is recommended every 3 years starting at age 6.



Rates are per visit and do not change if you opt out of services or if we are unable to complete a service due to age or behavior.



This is an In House Membership and not an insurance and is not a qualified health plan.



The membership plan benefits may not be combined with insurance, other offers or discounts.



Membership fees are due at the time of enrollment and expire 1 year from the date of enrollment. Enrollment is available on a yearly basis only.



All services rendered must be paid in full on the date of service.



Services are non-transferable and non-refundable.



Any services not used within the enrollment period cannot be carried over.



All fees shall be based on our practices usual, customary and reasonable fees, which are subject to change without notice.



As this is an “In House” membership, discounts cannot be applied to any specialist to whom we may refer.



Some exclusions may apply.



Plan will renew annually until cancelled. Annual membership price is subject to change.

I understand that these discounts are only applicable if I pay for services in full at the time of service; otherwise, I will be billed full price. I understand that my membership is annual and expires 1 year from the date of enrollment and my membership fee is non-refundable and all benefits are non-transferable. I understand that at each cleaning appointment I will be charged the discounted price regardless if all services included in the cleaning are completed, either because I have opted out or because they are unable to be completed due to the age and/or behavior of my child.
